Outreach & Social Services Descriptions
- Case Management: Short-term case management to provide referrals to appropriate agencies, assess needs, and match resources accordingly. Please call for an intake interview.
- Home Wellness Checks: We connect home-bound seniors with our Outreach Department to coordinate services to positively support seniors in need so they can age in place without risk.
- Fuel Assistance: This program is open to all residents of Brewster who qualify.
- Supplemental Nutrition Assistance Program (SNAP) food stamp information.
- Housing Assistance: We have a brochure for all housing on the Cape as well as the universal housing application. Call for details.
- Emergency Transportation and Emergency food shopping or prescription pick-up: Outreach offers this service on an emergency basis only. Call for more details.
- Nauset Neighbors: is a nonprofit organization that helps seniors stay in their homes and active in their communities. It offers transportation, social visits, computer help and odd jobs around the house, and connects volunteers and members across six towns on Cape Cod.
- The Telephone Reassurance Program: Administered by the Brewster Police Department at 508-896-7011.
- SHINE Program: The SHINE Program provides free health insurance information, counseling, and assistance to people who are eligible for Medicare and their caregivers. Certified, trained SHINE counselors, who are often volunteers, work with participants to help explore Medicare plan options and uncover ways to save and make sure you have the best insurance possible.
- Health Care Proxies, Five Wishes, Molst Forms: These are available, the most updated one of these being the Molst form.
- Elders of Concern Program: The Brewster Council on Aging prepares seniors for an emergency, such as severe weather, by compiling a list of community members that could use assistance during an emergency. This list is called Elders of Concern. Typical members of this group live alone, are disabled, use oxygen or other special medical equipment. Seniors can be added to this list by contacting the us and request to be added. Once on this list the Brewster Council of Aging or a member of Brewster Public Safety will contact these individuals prior to an emergency to ensure they are properly prepared and often after an emergency to ensure they are ok.

- SNAP (Supplemental Nutritional Assistance Program) The Brewster Senior Center has applications. Call 508-896-2737. Applications are also online at Mass.gov
- Project Bread's Hotline offers free confidential screenings for SNAP eligibility, as well as assistance in completing the application. Call 833-712-8027 for the dedicated Senior Line. Hours: Monday through Friday 8 am to 7 pm, Saturday 10 am to 2 pm.

Food Assistance
- Healthy Meals in Motion Monthly Mobile Food Pantry (Second Wednesday of each month)

Ext. 4226 for details. The Family Pantry is a non-profit organization located in Harwich, that helps the less fortunate of Cape Cod by providing food and clothing. - Brewster Housing Office The Housing Coordinator and Program Assistant hold open office hours on Thursdays from 10:00 AM to 12:00 PM for community members who have requests for assistance, questions, comments, and ideas about community housing. The coordinator is also available at other times and by phone, Call 508-896-3701 Ext. 1169 to set up an appointment.
- Lock Box Program: The lock box is installed next to your front door by Brewster Fire Department personnel who have exclusive access to the house key inside. Should you experience a medical emergency, the Fire Department is able to access your home and provide the help you need. Please call 508-896-7018
